Heart to the Rescue - KORDIAN 2020 Program - Program for the Prevention of Cardiovascular Diseases and Diagnosis of Familial Hypercholesterolemia in the Lodz and Mazovia provinces.
The preventive program of the KORDIAN project is preventive and educational in nature. The goal of the program is to increase the awareness and detection rate of cardiovascular diseases in people of working age. The program is aimed at medical personnel and patients.
Education of medical staff and personnel providing advice on diet, health education and physical activity – in addition to training on the assumptions and implementation of the program, includes education on the diagnosis and early detection of cardiovascular diseases, including cardiovascular failure, as well as strategies for dealing with individual risk factors, appropriate, effective communication with the patient with elements of increasing motivation to take effective prevention.
Patients eligible for the program are those of working age, not having been treated for a cardiovascular disease in the last 5 years (including at another provider), as well as not attanding the ChUK Program.
