TThe Project’s goal is acquiring and developing by disabled persons the possibly best achievable level of personal and social functioning through providing 108 persons who suffer from most severe disabilities with integrated social care and rehabilitation.It will be done through the development of social infrastructure in Zamosc and Kremenets and implementation in the two newly created centres of a system of Conductive Education which is to be a local modification of the system of integrated rehabilitation which will be worked out within the project.It will contribute to the improvement of life quality of the project’s addressees and their families.The Project is a continuation of an over 10 year period of cooperation with Ukrainian NGO’s.After professional preparation Ukrainian specialists by the Polish partner and organisational strengthening(creating an informal Ukrainian network organisation)we intend to stimulate the creation of model rehabilitation-social centres for persons with complex disabilities as part of the transborder cooperation programme.It will be done through working out a system of integrated rehabilitation as a method of work with persons with severe disabilities and adapting it to the local conditions.We also want to finalise the creation of the same system in the Zamosc area by providing support to disabled adults who are now cared for by their ageing parents.The following will benefit from the project: 80 disabled people from the Ukraine who are now deprived of any complex support,28 eight adults with complex disabilities from the Zamosc area who will gain access to support in the social cares home, and the therapy staff (50).The disabled persons will get support at 2 newly built centres which will offer new social services adjusted to the needs of persons with the most severe disabilities.The Project will satisfy the needs of 30 % of all persons with most severe disabilities who live in the areas of its implementation
