Proj.realiz.w okresie IV 2020-XII 2021 na terenie woj.mazowieckiego.Wsparciem w proj.objętych 150os.dorosłych (90 K,60 M) zamieszkujących woj.mazowieckiego,pozostających bez zatrudnienia,zagrożonych ubóstwem lub wykluczeniem społ.,tj.os.korzystające ze świadczeń pomocy społ.zg.z ustawą z dn.12.03.2004r. o pomocy społ.lub kwalifik.się do objęcia wsparciem pomocy społ.,tj.spełn.co najmniej jedną z przesłanek określonych w art.7 ustawy z dn.12.03.2004r. o pomocy społ. Celem gł.proj.jest podwyższenie do XII 2021r. aktywn.społ.-zaw.oraz zdolności do zatrudnienia 150os.niepracujących,zagrożonych ubóstwem lub wykluczeniem społ.,zamieszk.w woj.mazowieckim,poprzez objęcie ich kompleks.programem wsparcia realiz.we współpracy międzysektorowej, odpowiad.na zidentyfik.trudności i bariery,jakie napotykają w funkcjonowaniu społ.-zaw. W celu trwałego włączenia społ.i zwiększenia zdolności zatrudn.150 UP w proj.prowadzone będą kompleks.i zindywid.formy wsparcia,dopas.do potrzeb gr.docel.i uwzgl.najbardziej skuteczne działania z obszaru aktywnej integracji społ.-zaw.os.wykluczonych w ramach kolejnych zadań: ZAD.1 POGŁĘBIONA DIAGNOZA POTENCJAŁÓW I POTRZEB UCZESTNIKÓW PROJEKTU WRAZ Z OKREŚLENIEM INDYWIDUALNEJ ŚCIEŻKI REINTEGRACJI ZAD.2 AKTYWNA INTEGRACJA O CHARAKTERZE SPOŁ. ZAD.3 AKTYWNA INTEGRACJA O CHARAKTERZE ZAWOD. ZAD.4 AKTYWNA INTEGRACJA O CHARAKTERZE EDUK. Wsparcie każdego UP odbywa się na podst.ścieżki reintegracji i umowy na wzór kontraktu socjal.-w proj.oprac.150 INDYWID.ŚCIEŻEK REINTEGRACJI [dalej:IŚR] i podpis.150 ww.umów z UP.
