Proj.realiz.we współpracy międzysektorowej (sektor prywatny i pozarządowy) na terenie woj.mazowieckiego w okresie VII 2020-V 2022.Wsparcie w proj.skierowane jest do 200os.(120 K,80 M) w wieku pow.18r.ż.,niepracujących,które zamieszkują na terenie woj.mazow.,zagroż.ubóstwem lub wyklucz.społ.: *os.korzystające ze świadczeń z pomocy społ.[PS] zg.z ustawą z 12.03.2004r. o pomocy społ.lub kwalifik.się do objęcia wsparciem PS,tj.spełn.min.1 z przesłanek określ.w art.7 ww.ustawy *os.z niepełnospr.-min.10% UP *os.korzystające z PO PŻ-min.10% UP. Celem gł.proj.jest podniesienie w ramach współpracy międzysektorowej aktywności społ.-zaw. min.90% ze 200 UP (120 K,80 M),tj.os.niepracujących, zamieszkujących na terenie woj.mazowieckiego,zagroż.ubóstwem lub wykluczeniem społ.poprzez zwiększenie potencjału społ.i zaw.uczestników i uczestniczek proj.dzięki objęciu ich kompleksową ścieżką reintegracji(zaj.aktywnej integracji o charakterze społ.,zawod.i eduk.) do XII 2021r. Wsparcie każdego UP odbywa się na podst.ścieżki reintegracji i umowy na wzór kontraktu socjalnego-w proj.oprac.200 INDYWID.ŚCIEŻEK REINTEGRACJI [dalej:IŚR] i podpis.200 ww.umów z UP. W celu trwałego włączenia społ.i zwiększenia zdolności zatrudn.200 UP w ramach proj.prowadz.będą kompleksowe i zindywid.formy wsparcia,dopas.do potrzeb UP i uwzgl.najbardziej skuteczne działania z obszaru aktywnej integracji społ.-zaw.os.wykluczonych w ramach kolejnych zadań: ZAD.1 DIAGNOZA POTENCJAŁÓW IPOTRZEB UP WRAZ Z OPRAC.NDYWIDUALNEJ ŚCIEŻKIREINTEGRACJI ZAD.2 MODUŁ AKTYWNEJ INTEGRACJIO CHARAKTERZE SPOŁ. ZAD.3 MODUŁ AKTYWNEJ INTEGRACJIO CHARAKTERZE EDUK. ZAD.4 MODUŁ AKTYWNEJ INTEGRACJIO CHARAKTERZE EDUK. Proj.zakłada osiągnięcie wśród UP wskaźn.efektywności społ.i zatrudnieniowej w odniesieniu do 2 poniższych grup: 1) dla OzN-min.poziom efektywności społ.-34%,min.poziom efektywn.zatrudnieniowej-12% 2) dla pozostałych os.zagrożonych ubóstwem lub wykluczeniem społ.-min.poziom efektywn.społ.-34,min.poziom efektywn.zatrudn.-2
