CEL PROJ: integracja społeczna i aktywizacja zawodowa 70 os (42K,28M) zagrożonych wykluczeniem społecznym ze szczególnym uwzględnieniem OzN z woj. mazowieckiego do IX.2022 r. GD: 70 os (42K,28M) doświadczających wielokrotnego wykluczenia i/lub o znacznym lub umiarkowanym stopniu niepełnosprawności i/lub z niepełnosprawnością sprzężoną, które są zagrożone ubóstwem lub wykluczeniem społecznym, w tym 11 OzN (7K,4M) z obszarów poniżej progu defaworyzacji określonych w Mazowieckim barometrze ubóstwa i wyklucz społecznego z terenu woj mazowieckiego zg z KC. DZIAŁANIA: 1. Diagnoza indywidualnych potrzeb i potencjałów UP wraz z utworzeniem IŚR. 2.Panel kompetencji społecznych. 3. Szkolenia zawodowe podnoszące kompetencje/kwalifikacje 4.Staże zawodowe 4. Pośrednictwo pracy WSKAŹNIKI REZULTATU BEZPOŚREDNIEGO: L. os zagrożonych ubóstwem lub wyklucz społecznym, które uzyskały kwalifikacje/kompetencje zawodowe po opuszczeniu programu: 70 os (42K,28M) L. os zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu:12 os(7K,5M) Liczba os zagr.ubóstwem lub wykluczeniem społecznym pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek): 8 os(5K, 3M) L. os zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem w programie – 70. Liczba OzN objętych wsparciem w programie -11. Projekt zakłada osiągnięcie wśród UP wskaźników efektywności społecznej i zatrudnieniowej:1) dla OzN: min. poziom efektywności społecznej 34%, a efektywności zatrudnieniowej 12%; 2) dla pozostałych os zagrożonych ubóstwem lub wykluczeniem społecznym, min poziom efektywności społecznej 34%, a efektywności zatrudnieniowej 25%. Termin: 04.2021-09.2022. Miejsce: obszary poniżej progu defaworyzacji woj. mazowieckiego.
