CEL GŁ. PROJ.: Zaktywizowanie społ.- zawod. 54os niezatrudn.(min.30K) zagroż.ubóstwem lub wyklucz. społ., korzystaj.ze świadczeń pomocy społ. lub kwalifik. się do objęcia wspar. pomocy (w tym min. 16ON)- zamieszkuj. w gminach poniżej progu defaworyzacji woj. mazowieckiego (w tym min. 3os. z obsz. rewitalizowan.gm. Lipsko) -należące do minimum jednej z grup: os. doświadcz. wykluczenia wielokrotnego (min. 27os), os. z niepełnosprawnością sprzężoną lub os. niepełnosprawnych w st. umiarkowanym lub znacznym- poprzez realiz. w okresie 04.2021-11.2022 indywid. ścieżki reintegracji dla każdego uczest. GRUPA DOCEL.: 54 (min.30K) os. zagroż. ubóstwem lub wyklucz.społ., będących os.korzystaj. ze świadcz. z pomocy społ. zg.z ustawą z 12.03.2004 r. o pomocy społ. lub kwalifikuj. się do objęcia wspar. pomocy społ., będących os. niezatrudnionymi (100%, bierne zawod. lub bezrob.), należących (100%) do minim. jednej z gr: – os. doświadcz. wyklucz. wielokrotnego (min. 50% gr. docel.- min. 27os.), – os. o znaczn. lub umiark. stop. niepełnospr., – os. z niepełnosprawn. sprzęż. Ponadto os. z niepełnosprawn. (niezależnie od charakteru i stopn.)- stanowią min. 30% UP (16os)- kryt. prem. Uczestn.są wyłącznie (100%) os. zamieszk. (w rozum. KC) na obsz. woj. mazow.- w gminach poniżej progu defaworyzacji określ.w Mazow. barometrze ubóstwa i wyklucz. społecz.(zg. z zał. 9 do Regul. konkursu)- kryt. prem.; a wśród nich min.3os. to os. zamieszk.obszar rewitaliz. gm.Lipsko. WSPARCIE: zindywidualizowane dla każdego UP (formy udzielane w zależn. do potrzeb): IŚR, wsparcie psycholog./psychospołeczne, prawne/ obywatelskie/ finansowe, warsztaty umiejętn. społ., szkolenia zawod., staże zawod. i pośredn. pracy. EFEKTY: efektywność społ.- 34%, efektywność zawod.- 25% (ON- 12%), nabycie kwalifikacji/kompetencji- min.80%.
