Proj.realizowany w okresie 1 maja 2022 – 30 września 2023 terenie woj.mazowieckiego.Wsparcie w proj. skierowane do 60 os.dorosłych zagrożonych ubóstwem lub wykluczeniem społ.,w tym os.bezrobotne,kt.w pierwszej kolejności wymagają aktywiz.społ.Udział w proj.wezmą os.korzystające ze świadczeń pomocy społ.zg.z ustawą z dn.12.03.2004r.o pomocy społ.lub os.kwalifikujące się do objęcia wsparciem pomocy,tj.spełn.co najmniej jedną z przesł.określ.w art.7 ww.ustawy.Wśród UP min.36 K,min.9 OzN,min.20 os.biernych zaw.,max.40 os.bezrobotnych. Celem gł.proj.jest zwiększenie aktywności zaw.i reintegracja społ. min.80% spośród 60 (min.36 K) uczestników i uczestniczek proj.,tj.dorosłych mieszkańców woj.mazowieckiego,zagrożonych ubóstwem lub wykluczeniem społ.poprzez kompleksowe wsparcie realizowane w oparciu o zindywidualiz.ścieżkę reintegracji w okresie do III 2022r. Zad.realiz.w proj.: 1.PANEL DIAGNOZUJĄCY-diagnoza indywid.syt.społ.-zaw.UP wraz z oprac.Indywidualnej Ścieżki Reintegracji 2.PANEL KOMPETENCJI SPOŁ.-aktywna integracja o charakterze społ. 3.PANEL PORADNICTWA ZAWODOWEGO I POŚREDNICTWA PRACY-aktywna integracja o charakterze zaw. 4.PANEL KOMPETENCJI I KWALIFIKACJI ZAW.-aktywna integracja o charakterze zaw. 5.PANEL DOŚWIADCZENIA ZAW.-aktywna integracja o charakterze zaw. Każdy UP objęty diagnozą indywid.potrzeb i potencjałów w celu przygotowania i realiz.wsparcia w oparciu o ścieżkę reintegracji.W proj.podpisanych 60 umów na wzór kontraktu socjalnego. Gł.rezultaty proj.: -wzrost efektywności społ.wśród min.34% UP -wzrost efektywności zawod.wśród min.12% OzN i 25% pozostałych os.zagrożonych ubóstwem lub wykluczeniem społ. -liczba os.zagroż.ubóstwem lub wykluczeniem społ. pracujących po opuszczeniu programu (min.30%)-18 -liczba os.zagroż.ubóstwem lub wykluczeniem społ.,które uzyskały kwalifik.lub nabyły kompetencje po opuszczeniu programu (min.80%)-48 -liczba os.zagroż.ubóstwem lub wykluczeniem społ.poszukujących pracy po opuszczeniu programu(min.56% os.bier
