The Geriatric Center in Gdańsk
The Geriatric Center is created in the structures of the University Clinical Center by adapting the infrastructure to the needs of complex outpatient geriatric care. Preparation and implementation of project activities takes place in cooperation with the founding institution – the Medical University of Gdańsk. During the project implementation an interdisciplinary geriatric care center will be created, which will ensure cooperation of existing organizational units of University Clinical Center focused on providing optimal care of elderly patients. In particular, it will include medical clinic supported by specialist in internal medicine who also have a specialization in geriatrics, daily and outpatient rehabilitation and ambulatory outpatient services in the fields of cardiology and pneumonology. One of the main future objectives of the new interdisciplinary geriatric care center will also be improvement of cooperation with other medical professionals. The project will also contribute to implementing the fundamental principles of coordinated care and ensuring cooperation between various levels of medical service providers.
