The project is being implemented inpartnership between the City and Municipality of Sztum, on behalf of which operates the City and Municipal Social Welfare Centre in Sztum, and the “Gift of Heart” Association for the Chronically ill and the „Iskra” Social Cooperative. The project provided comprehensive support to 131 persons at risk of poverty and social exclusion from the area of the City and Municipality of Sztum. In the period between April 2018 until June 2020, families, senior citizen and persons with disabilities were given the opportunity to access a wide range of services, which resulted in an increase of permanent facilities within the social services delivery system. Under the project the families were offered support such as specialist counselling at a counselling facility which has been created, parenting workshops, family support worker, a social animator, self-help groups, a supporting family, holiday clubs for children and team building activities. The abovementioned support allowed families to overcome obstacles in care and upbringing matters, facilitated the acquisition of skills to properly manage the home budget, as well as demonstrated novel and alternative ways to spend leisure time together as a family. Moreover senior citizen were provided with comprehensive support such as care services and telemedical assistance services, whereas people with disabilities were provided with the support of a personal assistant, as well as with support of specialist care services. Under the project, there was an increase in the number of places at the Day Care Centre in Czernin, and the current attendees of the Centre were provided with access to a wider range of support services. Simultaneously, both the elderly and persons with disabilities had access to assistance and the company of volunteers. As part of the project, there was also training for the de facto custodians of the disabled persons as well as for carers of dependent persons.
