The project “Supporting Deinstitutionalization of Care for Dependent Persons” aimed to develop the healthcare system for dependent individuals, including the elderly. The project focused on improving the quality of healthcare services by preparing and implementing organizational solutions to develop healthcare provisions.
The main target group of the project were dependent individuals, particularly those over 65 years old, whose health condition required support but not continuous medical supervision. The project’s activities were centered around establishing a Day Care Medical Center (DDOM) for 25 places, catering to 120 participants, including 80 women and 40 men from Warsaw and its vicinity.
The scope of project activities included:
- Establishing the DDOM for 25 places by adapting premises and providing necessary equipment.
- Operating the DDOM, which involved conducting activities for 120 participants, organizing meals and medical transportation, and assessing the participants’ health status.
- Evaluating the health status of DDOM participants by specialists.
- Conducting educational activities for DDOM participants, as well as their families and caregivers, comprising 21 training sessions.
Despite the challenging period of the pandemic, the project was fully implemented, and its sustainability was maintained. Thanks to the project, participants were reintegrated into the local community. The tasks were carried out in accordance with principles of competitiveness, equal opportunities, and non-discrimination, including individuals with disabilities. The project was completed 100% in line with the schedule.
